Question: Q: problem -69842 scanning extSSD in Boot Mode

Running First Aid on my externally connected SSD (Thunderbolt v1) from within macOS (High Sierra v10.13.6 (17G3025)) it found no errors on the APFS formatted disk. However, performing the First Aid operation on the extSSD before macOS installed (usually it has to «freeze» the SSD to scan it from within macOS) I received the following output:

error: (old 0x269917) apfs_root: btn: dev_read(2544039, 1): Device not configured

The Volume /dev/rdisk5s1 could not be verified completely.

File system check exit code is 0.

Restoring the original state found as mounted.

Problem -69842 occurred while restoring the original mount state.

I am unsure what this means and cannot find information on the error code.

I suggest you re-prep the external drive:

Drive Partition and Format — El Capitan or Later

  1. Open Disk Utility in the Utilities’ folder.
  2. After Disk Utility loads select the drive (out-dented entry with the mfgr.’s ID and size) from the side list.
  3. Click on the Erase tab in the Disk Utility toolbar. A panel should drop down.
  4. In the drop down panel set the partition scheme to GUID. Set the Format type to APFS (SSDs only) or Mac OS Extended (Journaled.)
  5. Click on the Apply button and click on the Done button when it is activated.
  6. Quit Disk Utility.

Nov 13, 2018 11:46 AM

I can’t erase my main disk with OS.

Where did you get your information from please?

Nov 13, 2018 12:41 PM

You aren’t erasing the main disk. You are erasing the external disk with which you are having a problem. My information is from Apple and my 25 years experience with Macs.

Nov 13, 2018 1:08 PM

I am running my OS from the external SSD.

Nov 13, 2018 1:26 PM

Oh, I thought the external disk — » However, performing the First Aid operation on the extSSD» was what you posted. In order to erase it, you need to boot from the Recovery HD or from another disk. Or, you can try to reinstall macOS without erasing the disk, but it would only work if First Aid can repair the disk.

Reinstall El Capitan or Later Without Erasing Drive

Please be sure you back up .

  1. Restart the computer. Immediately after the chime hold down the Command and R keys until the Utility Menu appears.
  2. Select Disk Utility and click on the Continue button.
  3. Select the indented (usually, Macintosh HD) volume entry from the side list.
  4. Click on the First Aid icon in the toolbar. Wait until the Done button activates, then click on it.
  5. Quit Disk Utility and return to the Utility Menu.
  6. Select Reinstall OS X and click on the Continue button.

Install El Capitan or Later from Scratch

If possible back up your files.

  1. Restart the computer. Immediately after the chime hold down the CommandandRkeys until the Apple logo appears. When the Utility Menu appears:
  2. Select Disk Utility from the Utility Menu and click on the Continue button.
  3. When Disk Utility loads select the volume (indented entry, usually Macintosh HD) from the Device list.
  4. Click on the Erase icon in Disk Utility’s main window. A panel will drop down.
  5. Set the Format type to APFS (SSDs only) or Mac OS Extended (Journaled.)
  6. Click on the Apply button, then wait for the Done button to activate and click on it.
  7. Quit Disk Utility and return to the Utility Menu.
  8. Select Install OS X and click on the Continue button.

Nov 13, 2018 7:38 PM

Thank you for your help, but reinstalling the OS isn’t what I had in mind, it not really feasible on my main workstation either.

I was looking for a more surgical approach, to actually understand the issue.

Well, you can give this a try.

A Troubleshooting Procedure that may Fix Problems with macOS El Capitan or Later

You should try each, one at a time, then test to see if the problem is fixed before going on to the next.

Be sure to backup your files before proceeding if possible.

  1. Shutdown the computer, wait 30 seconds, restart the computer.
  2. Disconnect all third-party peripherals.
  3. Resetting your Mac’s PRAM and NVRAM
  4. Reset the System Management Controller (SMC)
  5. Reset your Startup Disk and Sound preferences.
  6. Start the computer in Safe Mode . Test in safe mode to see if the problem persists, then restart normally.
  7. Use Apple Hardware Test to see if there is any hardware malfunction.
  8. Repair the disk by booting from the Recovery HD. Immediately after the chime hold down the Command and Rkeys until the Utility Menu appears. Choose Disk Utility and click on the Continue button. Select the indented (usually, Macintosh HD) volume entry from the side list. Click on the First Aid button in the toolbar. Wait for the Done button to appear. Quit Disk Utility and return to the Utility Menu. Restart the computer from the Apple Menu.
  9. Repair permissions on the Home folder: Resolve issues caused by changing the permissions of items in your home folder .
  10. Create a New User Account Open Users & Groups preferences. Click on the lock icon and enter your Admin password when prompted. On the left under Current User click on the Add [+] button under Login Options. Setup a new Admin user account. Upon completion log out of your current account then log into the new account. If your problems cease, then consider switching to the new account and transferring your files to it — Transferring files from one User Account to another .
  11. Download and install the OS X El Capitan 10.11.6 Combo Update or 10.12.6 Combo Update or Download macOS High Sierra 10.13.5 Combo Update or Get MacOS Mojave now from the Mac App Store as needed.
  12. Reinstall OS X by booting from the Recovery HD using the Command and R keys. When the Utility Menu appears select Reinstall OS X then click on the Continue button.
  13. Erase and Install OS X Restart the computer. Immediately after the chime hold down the CommandandRkeys until the Apple logo appears. When the Utility Menu appears:
  1. Select Disk Utility from the Utility Menu and click on Continue button.

    sudo dd if = / dev / zero of = / dev / disk2 bs = 1m dd: / dev / disk2: конец устройства 61121 + 0 записей в 61120 + 1 записей из 64088969216 байт, переданных за 273592,584641 сек (234250 байт / сек)

  • та же проблема, ни одно из предложенных здесь решений не сработало, даже после diskutil unmountDisk /dev/disk2 я получил dd: /dev/disk2: Resource busy при попытке протереть его dd .
  • подождите . он начал работать после того, как я отключил устройство и снова подключил его .

Erasing and Partitioning Hard Drive from Internet Recovery

My MacBook Pro (mid 2012) was encrypted and running Yosemite 10.10.3. (The last update with Photos ruined it.) Yesterday, all of a sudden, everything freezes. I forced Shut Down and since then, it´s all gone.

I’ve tried every single possibility that I could find in apple support discussions forums. Tried to verify, repair, erase and partitioning the HDD and I just can’t. The only possible way to boot my Mac is through Internet Recovery (cmd+R, because there´s no Recovery HD left). I tried to erase the HDD through Terminal, with diskutil eraseDisk JHFS+ MacintoshHD /dev/disk1* .

*Sometimes the HDD I want to fix is disk0, others disk1. WTF!?

I had no success. I tried to partition it without success. When I try to re-install the OS or restore from Time Machine, the hard disk doesn’t appear. It’s like it isn’t mounted.

Is there a way to solve this without buying a new hard disk? I don’t mind going extreme, because I have backups. Is it possible it is something related with the fact that it was encrypted?

UPDATE: I tried every single possibility I could reach (e.g. GParted, TestDisk, tons of different command lines on Terminal, boot from USB, from CD). It seems to be dead.

Some brief example of errors

diskutil repairDisk /dev/disk0
Nonexistent, unknown, or damaged partition map scheme

Error repairing map: MediaKit reports bad partition or no map found (-5324)

Did you try erasing your disk with diskutil? You can do that via this command (assuming your disk is still disk2):

diskutil eraseDisk FAT32 NAME MBR disk2

Here is an explanation of the command:

diskutil: This is a disk management tool provided by Apple.

eraseDisk: Tells diskutil to erase a disk

FAT32: The filesystem format you want to use. You can replace it with any other filesystem, such as JHFS+ or FAT16

NAME: The name of the newly erased disk’s new partition

MBR: Partitioning table to use. You can replace it with GPT if you want.

disk2: Your disk’s identifier. Replace it if your disk’s identifier is different. You can find out your disk’s identifier using diskutil list

More information about the usage:

Usage: diskutil eraseDisk format name [APM[Format]|MBR[Format]|GPT[Format]]
        MountPoint|DiskIdentifier|DeviceNode
Completely erase an existing whole disk. All volumes on this disk
will be destroyed. Ownership of the affected disk is required.
Format is the specific file system name you want to erase it as (HFS+, etc.).
Name is the (new) volume name (subject to file system naming restrictions),
or can be specified as %noformat% to skip initialization (newfs).
You cannot erase the boot disk.
Example: diskutil eraseDisk JHFS+ UntitledUFS disk2

I have a new 250GB Crucial SSD installed in the original HDD bay of a Macbook (the SSD is not installed in the ODD bay). Through Parted Magic’s GParted, I created a GPT partition table but did not create any partitions. Then I get to Disk Utility by booting off of my Snow Leopard installation USB flash drive and attempt to create a partition. I get this error message:

Partition failed
Partition failed with the error:
Wiping volume data to prevent future accidental probing failed.

Furthermore, it wiped the partition table. If I got to the Erase tab and try to ‘erase’ the SSD drive as Mac OS Extended (Journaled) it will give me the same error message as above.

I have also tried the path of using GParted to create a GPT partition table and an HFS+ partition that takes up the full capacity of the drive. I get the same error as quoted above (in addition to the partition table and partition being blown away entirely).

If I attempt to format the drive using the Terminal utility in the OS X Installer using this command: diskutil eraseDisk JHFS+ Test /dev/disk0

I get this error:

Error: -69825: Wiping volume data to prevent future accidental probing failed

https://support.apple.com/en-us/TS2176
I have tried both Option 1 and Option 2 and get the same error as quoted above.

If I do: fdisk -i -a hfs /dev/disk0
Then go to Disk Utility, there is an unmounted disk0s1 that I’m unable to mount (if I click mount it turns gray for only a split second, partition remains a gray text). If I select the partition —> go to the Erase tab —> attempt to erase to Mac OS Extended (Journaled), I get this error:

Volume Erase failed
Volume Erase failed with the error:
File system formatter failed.

If I do the above again but skip the erase —> reboot instead to attempt to install OS X, the disk0s1 partition is not listed as a volume to install OS X on (but does still show up in Disk Utility).

I have tried the route of using GParted to create a GPT partition table —> boot from Snow Leopard installation USB flash drive —> Terminal —> gpt destroy /dev/disk0 —> Disk Utility —> create a Mac OS Extended (Journaled) partition, but get the error same error I have been getting (first quote in this post)

EDIT: I found success by removing the SSD —> plugging it in via USB adapter. I was able to create a partition using Disk Utility no problem. *sigh* soo stooopid

EDIT2: Turns out that the SATA cable is bad. Replacement @ http://www.amazon.com/Eathtek-Senso…_UL160_SR160,160_&refRID=1JQXGME162K5FBB1YSNC

Sorry — I didn’t mean to submit the thread without fixing that first, but I got carried away in writing my post and forgot about it (I was writing it up on the New Thread page and didn’t know what to title it).

I have this SD card that I am trying to use to backup photos on a Windows computer.

After using the SD card on the Windows computer, and copying the photos back on a safe drive, I cannot erase the SD card:

enter image description here

And yep, in the Info window for the card, the device is listed as «read only»:

enter image description here

I checked the Lock Tab and it’s not on Lock position… What gives?

A better and simpler solution would be using the command-line.

Execute the following command to identify the mount name of SD card following the pattern /dev/diskX, e.g., /dev/disk2

diskutil list

Say the disk name is /dev/disk2. Now format the card to FAT32 by running the following command:

sudo diskutil eraseDisk FAT32 MYSD MBRFormat /dev/disk2

I had to run

sudo diskutil eraseVolume exFAT MYSD MBRFormat /dev/disk2s1

to successfully format a volume on my 64G SD card.

For a reason I haven’t yet understood, the card was erased just fine once I used an older version of Disk Utility.

When I inserted the SD Card in a computer running Yosemite, two drives mounted, one called «NO NAME» which I could click on, and one called «UNTITLED» which seemed to be a ghost file or an alias:

enter image description here

When using Version 13 (606) of Disk Utility (so, the version included with Yosemite), the SD Card was erased without any problem.

The ghost «UNTITLED» drive disappeared.

I am therefore led to believe that the new Disk Utility in El Capitan doesn’t work the same way as the previous one, or that it failed at erasing the card in this particular situation.

I faced the same issue. I tried with 8GB, 16GB and 32GB cards and none worked. The trick was in the SD card reader. On the left side there was a switch for physical write protection. Toggling the switch and reinserting the card worked!!

I tried a different SD card reader, and that fixed the problem.
